HbA1c Loyalty Program Mechanics

Eligibility

·         All patients who avail of the HbA1c (GLYA1C) test are eligible to join the HbA1c

Loyalty Program.

·         Patients may avail of the HbA1c test even without a doctor's request.

·         Patients who agree to join the program will be registered through ZAP using their

active mobile number and will receive an HbA1c Loyalty Card.

Promo Coverage

·         This promo applies exclusively to the HbA1c (GLYA1C) test.

·         Valid only at the following participating branches:

o   Rockwell

o   Del Monte

o   Alabang

o   Las Piñas

·         Valid for cash and online payments only.

·         Not valid for HMO, corporate accounts, send-in, or home service transactions.

·         The free HbA1c test is non-transferable and not convertible to cash.

Loyalty Rewards

·         Patients will receive one (1) sticker for every qualified HbA1c test availment.

·         Upon their first (1st) qualified availment, patients will receive an SMS from ZAP

confirming their enrollment.

·         After their third (3rd) qualified availment, patients will receive an SMS reminder that

their fourth (4th) qualified HbA1c test is free.

·         Patients are encouraged to present their HbA1c Loyalty Card during every qualified

HbA1c test availment for sticker updating and personal tracking purposes only.

·         The Loyalty Card is not required for verification. A patient's qualified availments and

redemption eligibility may still be confirmed through the ZAP App using the registered

active mobile number, even without presenting the physical card.

Role of the ZAP App and the Physical Loyalty Card

·         The ZAP App is the official platform for tracking qualified HbA1c availments and

validating redemption eligibility under the program.

·         The physical HbA1c Loyalty Card serves only as a patient-facing reference and visual

tracker. It allows patients to personally monitor their qualified availments through the

stickers issued per transaction, serves as a reminder of the program mechanics, and

encourages patients to complete their succeeding HbA1c tests.

·         The physical card is not the sole basis for accumulation or redemption. Eligibility for

the free fourth (4th) HbA1c test will be verified through ZAP App records, coupon

redemption, and the corresponding ZAP Transaction ID.

·         If the physical card is misplaced, the patient's eligibility may still be validated through

ZAP, subject to verification of the qualified availments under the program.

Re-enrollment

Patients may re-enroll in the program after completing four (4) qualified HbA1c test availments, provided the promo period is still ongoing.

Promo Period

July 17, 2026 – July 16, 2027

The free HbA1c test must be redeemed within sixty (60) days after the end of the promo period. Unredeemed rewards after this period shall be deemed forfeited. Per DTI Fair Trade Permit No. FTEB-262854 Series of 2026
